The Missouri State High School Activities Association requires all student athletes to have a new physical on file each year before they practice with a team. Please fill out the physical form completely, including the insurance information on the back. It would be a good idea to make a copy of the physical form for your records. You can call the school to get a physical form or you may download one. You may also check 'physicals online' to see if the athletic department has a physical on file for you.
